Evergreen Healthcare Group is seeking a compassionate, relationship-driven Clinical Liaison to support our Billings and Laurel, Montana markets.
In this regional role, you'll be the vital connection between hospitals, physicians, families, and our skilled nursing communities—helping patients transition smoothly from acute care to post-acute rehabilitation and long-term care.

Build and maintain strong relationships with hospitals, physicians, discharge planners, case managers, and other referral partners throughout the Billings and Laurel markets
Evaluate referrals and assist with clinical screenings and pre-admission assessments
Coordinate admissions to ensure timely, seamless transitions into our skilled nursing communities
Serve as a trusted clinical resource for patients, families, and referral sources
Partner closely with facility leadership and admissions teams to meet census and resident care goals
Track referral activity and participate in strategic business development initiatives
Active LPN or RN license preferred
Clinical background in skilled nursing, post-acute care, hospital case management, discharge planning, or healthcare marketing strongly preferred
Excellent communication, organization, and relationship-building skills
Self-motivated with the ability to work independently across multiple locations
Willingness to travel regularly throughout the Billings and Laurel, Montana markets
Must reside within a reasonable commuting distance of the territory
